Welcome to the MLR 2010 Registration Form! The following directions will assist you as you complete your group's registration. If you have any technical difficulties, please contact Nick Magrogan at 973-761-0201 njmagrogan@salesianym.com. You will not be able to submit any forms without all of the required information! School Registration Begin by registering your school. You will need to provide a Contact Person that we can contact to verify and proofread your group's registration. Chaperon Registration You will have to submit the form once per each adult chaperone. Once you have submitted the form, re-click the "Chaperone Registration" link above to reset the form, which will allow you to register the next adult(s). Student Registration Just like the Chaperone Registration, you must submit this form once per each student. As you fill in the required information for each student, submit the form, then click the "Student Registration" link above to reset the form and you can continue registering your students. The Contact Person you list in the School Registration will be contacted by the Office of Youth Ministry to verify all the registration information that you submitted. Thanks! |
